I provide individual therapy to adolescents and adults, ages 14 and up. See below for my professional fees.
Insurance and Payment
I am an in-network participating provider with Tricare and Blue Cross Blue Shield. I will submit claims to these insurance companies on your behalf. You will be responsible for deductibles and copayments.
For clients with all other insurances, I am considered an “out-of-network provider.” If your health insurance carrier is willing to reimburse you for an out-of-network behavioral health provider, I will provide a detailed statement that you may submit to them. Please note that as an out-of-network provider, I will not bill your insurance company directly or accept payment directly from them. It will be your responsibility to pay me at the end of each session and to submit the claim to your insurance company. I suggest that all clients wanting to use their insurance benefits, contact their insurance companies about what their “out-of-network mental health benefit” is, so that you can make informed decisions. Please note that all insurance companies require a diagnostic code to provide reimbursement.
Payment
Payment in the form of cash, check, or debit/credit card is due at the time of service. Payment for Teletherapy services will need to be made via debit/credit card only.
Cancellation Policy
Please note that I require that you provide at least 24 hours notice if you cannot keep a scheduled appointment, so that I may offer that appointment time to someone else. If you are unable to provide 24 hours notice you will be required to pay a cancellation fee.
